The white wooden table located in the children's play are is splintering on two sides of the table and a screw is exposed. There is peeling paint on the left side of the exterior of the building.
One child with medication on the premises did not have a Medication Consent Form signed by their parent.
One child's Individual Health Care Plan did not include all of the required information.

During the visit, the fire drill log was not available for review. Please email a copy of the log to the licensor to review.
One child's Individual Health Care Plan included the administration of Benadryl. The Benadryl was not on the premises. Therefore, it was not immediately available if needed.
Educators did not take the attendance outside with them to the play area. Educators must have a method of knowing exactly who is present on the premises at any given point in the day.
The first aid kit did not include a CPR mouth guard. The licensee must maintain adequate first aid supplies, including, but not limited to adhesive tape, band-aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.

During the visit, two staff files were reviewed. Two files did not include evidence of the completion of the 12 EEC Essentials trainings. Bi-monthly observations of staff working with children were not documented.
The following hazards were observed outdoors and were accessible to children: the top portion of two large wooden tables were splintering and in need of repair. Four wooden garden boxes were in disrepair.
During the visit, an assistant teacher brought a group of 8 preschool children outside to the playground alone. An assistant teacher must work at all times under the direct supervision of at least a teacher-qualified staff person.
