One child's Individual Health Care Plan did not include all of the required information.
1. The Evacuation Procedures were not posted at the exit leading directly outdoors in Classroom 6. 2. Evacuation Diagrams must be posted at each classroom egress that does not lead directly outdoors.
In Classroom 3, a teacher's purse was accessible to the children. In Classroom 10, plastic packages of wipes and a mop were accessible to the children. There were several stained ceiling tiles in the front hallway.
