The program's Building and Fire Inspection Certificates were not current. During the visit, the Director stated they were waiting for the town inspector to schedule an inspection visit.
A review of two Individual Health Care Plans indicated that the both did not indicate which staff were designated and trained to administer emergency medication.

The following was observed and accessible to children: BATHROOM - The trash container was not covered. PRESCHOOL- One of the window screens was torn and not in good repair. The Program Director stated that it is in the process of being replaced; a corner of the Dramatic Play shelf was cracked and had exposed wood, cause for a potential injury.

The following was observed and accessible to children: PRESCHOOL/SCHOOL AGE - The play kitchen was unsecured and tippable. INFANT/TODDLER - The circle rug was curled up and a potential tripping hazard; The play kitchen was unsecured and tippable; There was a small hole in the half wall; One of the crib mattresses had a rip. INFANT - The crib in the left corner had chipped paint on the headboard.
