During the visit there were times when children were outside and children were not being safe and the licensor had to bring attention to the unsafe behaviors for educators to get the children to stop the behaviors. At one point the licensor had to ask the children to stop before a child was seriously hurt as educators did not see what was happening.
Allergies, medical info and other information regarding children's medical needs were not posted in a manner that protects the privacy of children or in a manner that could be accessed by educators that were working in the program.
Children’s records were missing IHCPS. There were also children attending the program that are not enrolled at the program. Progress reports were not in the files for review.

Two educators [redacted] reported that their EEC essential trainings are not completed. Those educators reported that they have been employed at the program[redacted].
Effective communication was not happening during the licensing visit, educators while in the cafeteria were not engaged in positive interactions and child guidance practices.
At the time of the visit there were no interest areas set up at program to foster independence and self help skills. There was no evidence of curriculum and daily activities.
There was no equipment set up for the children when they arrived at the program. Materials in the cabinet could were not accessible to the children as the cabinet was faced toward the wall. Materials could not be seen by the children.
There was not written evacuation procedures or a map posted near the exit or on a tri fold board at the time of the visit.
Staff reported that they are not practicing fire drills on a monthly basis.
During the visit when the children were lined up to wash their hands before snack, the children were confused as to why they had to wash their hands, they were looking at each other confused saying "we never do this".
There were children who required medication that did not have their medication available at the program.
The space that the children were playing in was not completely fenced in and children were sitting near the door to the school and not supervised always supervised and could have gone to the road if they wanted to and no one would have known as the staff were focused on the playground. Where the children were located was adjacent to the road.

During the visit the outdoor playground did not have sufficient mulch to serve as impact absorbing material in several areas that were measured. The protective coating on the climber was peeling and could pose a risk to children.

During the visit there was an inhaler that was not stored in the original box with the prescription label affixed to it.
During the visit it could not be confirmed if attendance was accurate and when the site coordinator was asked how many children were attending the program they could not give the number of children attending. At one point the site coordinator was in the cafeteria with approximately 33 children while the other staff were with children outside of the cafeteria.

During a file review an IHCP was missing for a child that had a listed allergy on their paperwork. 5 files were reviewed, 2 with an IHCP but one child who requires an IHCP was missing their IHCP.
During the visit educators were not engaged in positive interactions as there was a lot of yelling to get the children's attention that was not resulting in positive child guidance practices. Children were yelling at each other without the intervention of educators to gain control of the children. Effective communication was not happening during the visit that was leading to more chaos during the transition from snack to outdoor time.

During the visit there was no opportunities for the children to foster independence and self help skills which would lead to evidence of curriculum as there were no materials available for the children during the visit.
During the visit there was a posting about allergies and medical information but it was missing what children had the allergies/medical conditions and what medications are required for those allergies/medical conditions.
There was a child attending the program who did not have their emergency medication at the program.
A current staff schedule was not posted at the time of the visit.
The slides had exits that were above the required 11 inches and under. All exits to slides must be no more than 11 inches above the protective surfaces (twist slide was 21 inches, the bumpy slide was 23 inches and the straight slide was 24 inches).

A current staff records checklist was not available for review at the time of the visit.
During the visit there was 32 school age children with two educators. Director reported that other staff were coming in but that there was going to be a gap and that was observed during the visit.
At the time of the visit the playground had areas that were not properly maintained. The playground in several areas did not have the required amount of mulch for the impact absorbing materials. Under the swings there was only 3 inches of material. Most other areas had 8 inches and by the rock wall there was 6 and a half inches. The requirement is 9 inches.
This investigation inspection recorded no violations or advisories.
