The licensor noted the following on the playground: 1. There is a metal platform( that was previously attached to a outdoor play item) that was exposed and would be a tripping hazard. 2. The plastic/rope covering on the ropes attached to the structures are breaking which could be a potential hazard. A child could cut their fingers on the broken pieces.
Upon review of childrens files, the following was noted: 1. An off-site consent form was not signed. 2. Two transportation plans were not checked how the children will arrive or depart.

The fire drill log did not document the effectiveness of the drills.
Upon review of staff files the following was noted: Two staff members did not have a program orientation form in their file.
Two children who were transitioning from the Toddler 2 Classroom were not signed out.
After a child had already put snack on their plate, the teacher informed them that they needed to wash their hands. However, the teacher first asked the child to pick up the activity they were participating in and then could return to snack. Upon return to snack, Child A was again reminded to wash her hands after she had already went and picked up her snack plate.
