A review of six children’s records showed that three physicals were expired, two enrollment packets were expired, and one child was missing lead documentation.
During the visit, it was observed that the educator was using a cooking thermometer in the refrigerator, which is not appropriate for monitoring required refrigerator temperatures.
During the visit, it was observed that one of the designated exits in the basement did not have an exit sign posted.
During the visit, it was observed that all napping materials and sheets were clumped together in a closet and not stored separately, and the educator was unable to identify which items belonged to each child.
The educator currently has a dog in the program but was unable to provide vaccination documentation at the time of the visit.

During the visit, it was observed that the first aid kit was missing a CPR mouth guard and a thermometer.
During the visit, it was observed that the educator did not have a second surface available for diapering and toileting.

Records were reviewed for the 7 enrolled children. Six of the children needed proof of current medical documentation.
The progress reports were not current for the enrolled children. The last proof of progress reports for some children was over a year ago.
There was a slide in the licensed space that exceeded 18 inches in height and did not have appropriate ground surfacing. Children were observed climbing the slide and jumping from the top of the slide. The Educator did move the slide after EEC Licensor informed her that the slide could not be used with appropriate ground surfacing.
