Upon review of the attendance records, it was observed that the attendance sheet did not accurately reflect the children present on the day of the visit.
During the visit, it was observed multiple pieces of wood were scattered in the backyard.

The hot water in the childcare bathroom was measured to be 130 degrees. Discussed the temperature must be less than 120 degrees.

Progress reports were reviewed for the 6 enrolled children. One child needed proof of a current progress report.
The records were reviewed for the 6 enrolled children. One child needed an Annual Update and proof of current physical and immunizations.
The evacuation log forms had not been completed.
The educator did not have proof of all professional development training hours. She had 22 hours available for review. Please send proof of 8 additional hours.
There was one enrolled child with a documented allergy. This allergy was not posted.
The refrigerator temperature was 50 degrees. Discussed the temperature must be between 32 and 40 degrees. Please send proof of correct refrigerator temperature.
There was a shelf unit that was a tipping hazard in the basement childcare space. In the outdoor playspace, there was access to gardening equipment, tools under the deck, and large glass vases.

There was an assistant applicant on the premises when Licensor arrived. The Educator reported the applicant had stopped by to drop off paperwork work and to get help with accessing the LEAD Portal. This person did not have an approved background record check.
The Educator installed a large swing/slide/climbing structure. The structure does not have 6 foot fall zones as required.
The times of arrival and departure had not been documented for several days. Discussed the times of arrival and departure must be documented as soon as children arrive to and depart from the program. The Educator has been previously cited for not having accurate attendance records.
The exit to the back yard was blocked by a doll house inside and gardening equipment outside. The Educator has been cited previously for the dollhouse blocking the exit.

The Educator has created her own enrollment packets. Reviewed that all required EEC information must be included in the Enrollment Packets. The Educator did not have the information for the General Permission section in her packets. One child was missing proof of a lead screening.
