When the licensor arrived she observed a portable space heater being used in the licensed space. The educator removed the space heater during the visit at the licensors request.

A review of the children’s records show that 2 children were missing an annual update, 1 child needs an updated medical, 1 child did not have a medical on file, 2 children’s general permission were blank and 1 child did not have the physical description on pg. 1 of packet
A review of the staff records did not show training certificates for the assistants
The educator is documenting the arrival/departure times for the children in care. The attendance for the assistants is not accurate. 2 of the assistants have not documented attendance since July/August
The educator is not always documenting the how many chidlren were present, what exit was used and how long the fire drill takes
The parent handbook provided to the licensor was missing: lead paint statement, child guidance statement, medication policy
One of the kitchen drawers did not have a lock and the licensor observed a screwdriver and a lighter accessible to the children
