During the visit, 3 children's files were reviewed. One child's physical expired 11/20/2025. One child's file did not include evidence of a Lead test.
At the time of the visit, the following was noted. Educator's First Aid certification expired 1/2/2026. All educators must obtain First Aid Certification within six months of employment, and must maintain thereafter.

During the visit, three children's files were reviewed. One file did not include a current Transportation Form. One file included a First Aid Emergency Medical Form that was not signed and dated by the parent. One file did not include a current Developmental History Form. Three files did not include a current physical. One file did not include documentation of immunizations or a lead test date.

A review of the Fire Drill log indicated that practice drills were not conducted in February, March, April and May of 2025. The licensee/educator must hold practice evacuation drills with all groups of children and educators from each floor level of the approved space at least monthly. (This was previously cited on 6/7/23 & 5/15/24)
During the visit a teacher's purse was observed on the floor and accessible to the children. The purse was moved during the visit.

A review of the program's Fire Drill Log indicated that a practice drill was not conducted in September 2023 and April 2024.

During the visit, three staff files were reviewed. Two files did not include a current physical. Bi-monthly written observation of staff while working with children were not conducted
An electrical power strip did not include safety covers over the receptacles not in use, to prevent the children from accessing them.
Fall zones adequate depth of impact absorbing material
The yellow see-saw and the gray castle climber did not have a 6-foot fall zone around all sides of the structures. .

Staff meetings have not been conducted monthly as required. In programs with four or more staff the licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of the regulations; children's behaviors and meeting the individual needs of children.
The paint was peeling on the roof fascia board located above the playground. This was previously cited on 11/8/2021
Evacuation drills have not been conducted monthly as required. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
The following was observed outdoors. The gray climbing structure was located next to the fence and did not have the required 6 feet fall zone. The green see saws and the brown see saw did not have the required 6 feet around the fall zones. The two plastic kitchen sets were not secured.
The following was observed indoors and accessible to the children. A bookcase located near the rear exit was not secured. The wooden doll house was not secured. The white table with the fish tank on it was not secured. The wall under the storage door is in need of repair. There was food splatter on the wall behind the garbage barrel.
During the visit, two staff files were reviewed. Two files did not include documentation of a PQ Registry. One file did not include evidence of immunity to Measles, Mumps, and Rubella.

Physical Space Paint
Peeling paint was observed on a window sill in the classroom. The paint is peeling on the roof fascia board located above the playground.
The location of the first aid kit posted in the classroom was not accurate.
