A sample of staff files was reviewed. One staff file did not include 2 doses of measles and mumps or proof of immunity.
A file that was reviewed for one child included that the child was allergic to a medication. That information was not noted on the Allergy/Medical Condition List.
One child's Medication Consent form was signed over a year ago and has now expired.

The handwashing sink shared by the Infant 1 and Infant 2 classrooms had a water temperature of 137F. Water temperature must be 120F or under.
The MMZ classroom did not include written evacuation plans at each exit and an evacuation diagram at the door that does not go directly outside.

Kitchen Barrier
One child's file included two Medication Consent Forms that did not include all required information.
At the time of the visit, the door to the kitchen was open. There must be a barrier between children's activity space and the kitchen.
Two children's Individual Health Care Plans did not include which staff were trained to administer the emergency medication and who trained the staff in the administration of the medication.
There as a small area on a wall that had peeling and chipping paint in the Toddler 2 classroom.

A sampling of staff files was reviewed. One staff file was missing a bi-monthly observation.
1) The Staff Meeting Log did not include a monthly staff meeting in January and April '24. 2) Staff meeting documentation for several months did not include the start and end times of the meeting. Therefore, EEC was unable to assess if staff were meeting for a minimum of 2 hours monthly.
In the Preschool Classroom, 4 wooden shelving units were tippable. Please email EEC photos of these items made non-tippable,
The first aid kit reviewed did not include scissors.
The Fire Drill Log for 2024 did not include evacuation from alternative exits.
1) One child's Individual Health Care (IHC) Plan did not include possible side effects of the medication and consequences if the treatment is not administered signed/dated by the doctor. The parent had not signed/dated the plan that included the staff who were trained in the administration of the medication and who trained the staff. 2) One child's IHC Plan included a dosage of 1 teaspoon (5ML). The Medication Consent Form included a dosage of 2 ML. This information needs to be consistent.
Two children's files did not include Medication Consent Forms completed by a parent for administration of the child's emergency medication.

1) [REDACTED] educator file did not include a completed program orientation check list, evidence of bi-monthly staff observations and sufficient professional development hours for the past year. 2) [REDACTED] educator did not include the on-line Intro. to EEC training certificate and evidence of bi-monthly observations.
At the time of the visit, the licensor observed an infant sleeping in a crib. The pacifier had come out of the child's mouth and had not been removed from the crib.
In the Infant 2 classroom, [REDACTED] infants time of arrival had not been recorded on attendance. This did not ensure that the program had a method of knowing which children were present at all times.
In the Infant 2 classroom, the tops of [REDACTED] infant bottles were not labeled with the child's name for who they are intended.
There was a small amount of chipping paint on the walls in the Infant 1, Infant 2, Toddler 2 and Preschool classrooms.
At the visit, the licensor observed [REDACTED] infants in Infant 1. The educator reported that [REDACTED] other infants from that class had been moved to Infant 2. The educator coordinator confirmed that this was done due to insufficient staffing. The licensee did not maintain appropriate staffing at all times, including when regularly assigned educators are absent due to illness, personal business or vacation.
At the visit, the licensor observed [REDACTED] infants in Infant 1. The educator reported that [REDACTED] other infants from Infant 1 had been moved to Infant 2. Children were not assigned to a consistent group with consistent staffing.
[REDACTED] children with [REDACTED] conditions did not have complete Individual Health Care Plans (IHC). Items missing were potential consequences if medication not administered, who was trained to administer medication and trained staff to administer medication.
1). [REDACTED] children’s medications were not accompanied by an Authorization for Medication Form. 2) [REDACTED] child's medication dispenser did not include the specific dosage required by the doctor.
