One staff file did not have documentation of a staff information form, and an expired CPR form.
One children's file was missing EEC required information on their First Aid Emergency Care Consent form. Two children’s progress reports did not indicate the dates of the actual report.

Two children did not have documentation of a current progress report. Three children's enrollment forms and first aid emergency consent forms were missing EEC required information. Three children's development history was expired.
