Current consent for administered medications
One child has a medical condition for which medication was prescribed but the parent has not provided the educator with the consent for administered medications. Educator stated that she has provided the parent with the permission for medication administration form and has reminded the parent that she needs to complete the form and return it to her.

First aid kit is missing emergency services numbers as well as a thermometer.
There is a metal swing set in the backyard. The metal swing set is missing adequate fall zone and a protective surfacing. A snow blower was observed in the back yard, accessible to children and could be a hazard. Chipping paint was observed on the second emergency exit door frame leading to the backyard.

Indoor Hazards
At the time of the visit, porch door has no visible locking mechanism or way to keep the door firmly closed. This allows for easy exit and access to stairs.
An enrolled child does not have a well child visit on file. -A child with medical condition requiring life saving medication has no IHCP plan on file and no medication on the premises.
Sharp corners in living room and playroom are unprotected.
