At the time of visit the licensor noted the following missing items in a child’s file who had an individualized health care plan: 1. The child’s file did not include location of the of walking field trips and offsite playground. 2. The child’s individual health care plan did not indicate who can administer medication to the child, and who trained the educator(s) who can administer medication to the child. 3. The child’s file has incomplete injury reports. 4. The child’s file had missing progress reports. 5. The child’s lead screening is not current.
At the time of visit the licensor noted that a child’s potty seat was on the floor of the preschool bathroom floor, and that the broom and dust pan was accessible to children.
