The swing set, which was cited at the previous visit, is a hazard to children, does not have adequate fall zones, and has not been removed.
The toothbrush for the one child who requires it is being stored covered, which was cited at the previous visit.
One child is missing an updated Enrollment Packet.

At the time of the visit, a metal swing set was observed in the outdoor play space, without adequate fall zones.

At the time of the visit, the child appropriate activities/materials were not visible nor accessible to children.
There is a metal swing set and slide in the play yard outside which is a hazard to children. The Educator stated she replaced her old swing set with this new one in December.
At the time of the visit, the Educator allowed an Infant to sleep in the car seat while wearing a bib and with a blanket covering the front of the bucket seat.
The TV in the 2nd playroom is not secured and is a tipping hazard for children.

The Educator is missing updated Progress Reports for all children. The last Progress Reports for some children were completed in Dec. 2018.
The Educator stores toothbrushes, for the children who require them, so they are not exposed to the air and touching each other in a small container.
