One child's medical record is due for update; child's last physical was 02/20/2025. General Permission are not completed or signed. Educator is using the Special excursion permission form for regular outdoor activities. Licensor reviewed and explained it to the educator and educator stated she will redo it.

Backyard: there are holes on the ground that can become a tripping hazard, there's a trampoline in the backyard and it is accessible to the children, there is an opening area on the side of the garage, and it is not barricaded. There is a pump with pipes coming from underground, and the pipes are accessible to the children.

The educator has not replaced the fence for the pool. This was reviewed with the educator on 10/15/2024 that current fencing was not an appropriate barrier for the inground pool.
This technical assistance visit inspection recorded no violations or advisories.

Children’s files were reviewed. Two files did not have an annual update and one file did not have an up to date medical physical on file. One file has a nutrition plan that needs to be updated by specialist.

It was observed that on one side of the daycare room, although there was a gate installed to prevent access, it is licensed space. There was a portable ring light stand that was easily tippable. It was observed that the white toy shelf was easily moveable and there was a portable back drop prop that was a tipping hazard as well.

It was observed that stairways leading up to firs floor level emergency exit had large openings on each side of the railings where children could fall over.
This follow-up visit inspection recorded no violations or advisories.
Licensor observed space where infant was napping was too dark and did not have appropriate lighting for proper supervision.

It was observed that the fencing installed to prevent access to the in ground pool from the basement level, was observed to not be sturdy from one of the sections. Educator must ensure gate to prevent access to the pool is always secure and sturdy.
At time of visit educator stated she had a (REDACTED) napping. Licensor observed a small security blanket in the pack and play where infant naps and the sound machine was inside the pack and play as well. Licensor reviewed safe sleep regulations and practices with the educator. Educator was informed that parents will need to sign the safe sleep parent letter, educator will need to sign the safe sleep pledge and retake the safe sleep training. This will be emailed to the educator. A follow up visit will be conducted.
Educator would like to use the side of her backyard for additional play space. It was observed that there was access to multiple boxes with a variety of items that are to be disposed. There was access to a canopy stand that is easily tippable and a coat hanger that is also easily moveable. There was outdoor patio furniture that was stacked one on top of the other. Educator must add permanent barrier at the top of the basement stairs.

Daily was observed to only have two children signed in and there were three children present total.
Review of PQ registry showed educator’s PQ as expired as of 12/2023.
Today’s attendance only had one child signed in when there were 4 children total.
One child did not have an updated progress report on file.
Educator has a child who still eats all meals in a bottle. Child was observed to be taking bottle in a bouncy chair that has an incline this is a chocking hazard.
Children’s records were reviewed. Three files did not have lead screening results on their physicals. One file had missing parent signature and date on page 5 of enrollment packet and on page 7 of enrollment packet educator’s name was missing.
Educator has created a personalized parent handbook, however it does not include all required information EEC requires.
There was access to plug-in air fresheners. Educator did remove plugins during visit. It was observed that there were multiple cords accessible, cords observed were from cell phone charger, computer cord, and cord from a lighting accessory. Educator did put away the lighting accessory and moved cords where they were not accessible to the children during the visit.
