Educator left a 5 month old infant alone, who had just started in the program, in a bouncy seat while she and the assistants went into other rooms of licensed space.
The linoleum floor in the closet in the Playroom is broken and coming up in places. The linoleum floor in Napping Room 2 is broken along the doorway and around the wall by the door. Assistant's backpack was on the floor of the closet in the Playroom, where children could access it. Home has mold and dust in the windowsills and a thick layer of dust along the baseboards.
The assistant stated that only the small area in front of the car parked in the driveway is used as the outdoor play space. The space is insufficient space for 10 children.
A pack n play used for an infant has rips on the bottom of it and is not in good condition.
An infant was observed sitting in a bouncy seat for the entire 2 hour visit until it was time for the infant to eat at the end of the visit.

Licensor observed [REDACTED] 11 months old infants sleeping behind closed doors.
The exit route through the kitchen is icy and can become a potential hazard in an emergency evacuation.
The diaper changing pad is not impervious to water or easily cleaned. It is covered with a cloth.
One Child was missing an emergency card . Vests used to identify the children when they are taken off of the premises is missing the address of the program.
2 life saving medications for 2 children who require them are expired.
A window in the bathroom is missing a screen.
A radiator cover in the Kitchen and one in the bathroom are not secured.
