All three staff files were missing evidence of bi-monthly observations and annual evaluations. 2. One educator was missing evidence of MMR immunizations.

Feeding schedule for each infant/toddler; Infants held while fed from bottle
At the time of the visit in the infant/toddler 1 room, EEC observed 4 toddlers sitting at a table, not engaged in an activity. They were watching a show on an IPad. The curriculum posted was not reflective of the activity nor the daily schedule. The PD reported during the exit interview that staff are not allowed to use the iPad in the classroom for children.

1. One toddler/preschool classroom had 11 children napping but the attendance had 8 children listed. 2. Three children that transitioned from the older toddler room to the toddler preschool room were not signed in on the attendance and were not signed out from their toddler room.

The fire drill log did not document alternate routes being used.
The cheerios in the Infant Classroom were taking out of the original package and did not have an expiration date on it.
Teachers in the Infant Classroom and in the Toddler Classroom did not wash their hands or the child's hands after wiping a child's nose.
A child's hands were not washed prior to being put into a highchair to eat.

One child's medication was not maintained at the program.
The transportation registration was not available for review at the time of the visit.
1. A teacher in the Infant/Toddler Classroom left the room to put away something in the closet, which left the classroom out of ratio. 2. When a teacher was helping a child in the bathroom, from the Preschool/School-Age Classroom, the children in the classroom were not visible to the teacher.
The furniture was not secured in the Pre-School/School-Age Classroom.
The program did not have a staff record checklist that indicated that the driver had a current license.

It was observed that there is chipped paint throughout the downstair classrooms.

Two children's individual health care plans did not contain all required documentation.
The attendance log did not document that a second person signed off the form to ensure that no children were left on the van.
The program had attendances logs documented for October and November but not prior.
