All three educators were missing evidence of bi-monthly observations as required.
1. At the time of visit, K1-4 room did not have an attendance roster in the classroom. 2. One child was not signed in on the roster in Nursery 1 on 1/22/26. 3. Over ten children were not signed out in K1-3 on 1/21/26 .
One medication was expired 11/25.

One child on two medications was missing a consent form for one of the medications.

At the time of visit the mats in three classrooms were not 2 feet apart.
