The following were observed during the visit: -A trash can with food waste did not have a lid. -One shelf in the back preschool room was tippable. -Two cots were cracked on the hard plastic corner protectors, creating a pinch point.

The SA room was on the small playground at 9:40 AM. Staff reported that they did not have the attendance record with them, as it was charging. The staff did not have an alternate method of tracking attendance with her, outside.

On the playground a hose was draped on the ground, causing a potential trip hazard. Children were running through this area at the time of the visit.
