Safe Sleep - cribs have proper mattress
The port a crib designated for the infant was not considered safe. There was an additional mattress not secured in the port a crib on top of the original mattress provided by the manufacturer. The original mattress had gaps along the edges of the crib. In addition, there were holes in the sides of the port a crib, a potential entrapment hazard.

There was an Epi pen medication for a child that was not in the container in which it was originally dispensed and with the original label affixed.
