It was observed that two out of nine medications was not stored with their manufacturer's packaging and prescription label affixed.
A review of nine medication consent forms indicated that one was not fully complete to include the following: whether or not the medication was a prescription and if it was previously taken, the dates and times for when the medication should be administered.

In the Group X room, there were cords over 6 inches long accessible.
When the group moved to the Group X room, the attendance was left in the main room. The program must a method of knowing exactly who they are assigned to supervise.
