Evacuation procedures were not posted next to each exit in the preschool and school age rooms, and one evacuation route and procedure was missing next to the door leading to the front parking lot in the preschool room.

Playground free from hazards; fenced if applicable
Individual Health Care Plan (IHCP) - R Apr 2025
It was observed that there is a hazard (large hole) in the fenced in preschool playground, and that one area of the school age play area is not fenced in near the parking lot. The outdoor play area must be free from hazards including but not limited to: a busy street and a parking lot. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.
During a review of children's files, it was found that two children were missing information on the doctor's IHCP. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered. It was recommended that the program attach the EEC IHCP to the doctor's plan to ensure that it contains all required information.

During the visit, it was found that a TV was balancing on a small shelving unit and not mounted on the wall, which poses a hazard and may cause an injury to a children.
During the visit, it was learned that a child's Epi-pen was expired.
