While looking at the Toddler classroom attendance they had four children singed in, six children were in attendance. The educators reported they had paper attendance and electronic attendance. Both had inaccurate attendance during the classroom observation.

During the infant/toddler classroom observations the window sills and floor boards had chipping paint. One wall had peeled paint from taking down art work.

Sufficient materials & equipment visible, available, and accessible
It was observed that the infant equipment in the outdoor space, including an exersaucer and infant seat, were dirty and not properly maintained.
The preschool room did not contain a sufficient quantity and variety of materials, including art, books, music, science, blocks and accessories, and sand/water/sensory materials that were visible and accessible to engage all children present in the program.

It was observed that the wood fence surrounding the toddler playground is not in good repair. The fence is deteriorating and has sharp edges along the bottom of the fence in several areas.
The wood chips in the fall zones under and around the climbing structure in the outdoor space did not contain an adequate depth of impact absorbing material. The wood chips were compressed and measured approximately 5 inches in two areas.
It was observed that the program did not provide sufficient shade in the outdoor space when a mixed group of six infant/toddlers and three educators were huddled together in a small area to protect against the sun.

During a review of children's records, two children were missing parents' signatures on Individual Health Care Plans, one child was missing a medication consent form for non-prescription medication, and two children were missing current physicals.

Safe Sleep
During observation and inspection of the infant/toddler room, the Licensor noticed that one portacrib did not have a snug fitting sheet and an infant was placed in the portacrib with the loose fitting sheet.

Several cabinets and drawers containing hazards were unlocked and accessible to children, a pocketbook was stored at children's level in the preschool room, and a climber measuring 21 inches did not have a fall zone.

Emergency medications readily available
During a review of children’s records, it was found that (3) children are missing current enrollment paperwork, (2) children are missing current physicals, and (4) children are missing medication consent forms for chronic medical conditions.
An educator working alone in the infant/toddler room does not have the required work experience (3 months) caregiving for infants and toddlers.
An inhaler was missing for one child in the preschool room, and there was no written documentation that the child no longer requires the medication while attending the program.
This investigation inspection recorded no violations or advisories.

Equipment is clean, safe, secured, and poses no hazard.
At the time of the visit there were no chairs in the toddler room for the group of children to sit on. Licensor was told that the chairs were put away to prevent the children from pushing them around the room. Educators put some chairs out during the observation but not enough for the number of children present.

A review of children’s records revealed that some of the Individual Health Care Plans (IHCP) were missing information.
A service provider was observed taking the child to the bathroom with no direct supervision from a teacher qualified Educator.
At least two shelving units in the toddler room were not secured from tipping and could pose a danger for children.
There was a large Television in the infant/toddler room that was showing a children story for toddlers to watch. Daily activities offered must be developmentally appropriate for children.
On the day of the visit Licensor noticed that there was a small refrigerator and a microwave plugged into a power cord. This may be a possible fire hazards.
The location of the Health Care Policy was not posted on the day of the visit.
The crib designated for use during evacuations did not appear to be constructed for the purpose of bring used as an evacuation crib. Evacuation cribs must meet standards.
The changing surface in the toddler room was torn in some areas preventing proper cleaning/disinfecting. This could pose an issue with cross contamination.
On the day of the visit there was bag on the floor with a child’s belongings that could prevent the safe evacuation of children during an emergency.
The attendance recorded on Pro-Care included a child that was not in attendance at the time of the visit.

Medication

A child diagnosed with three chronic medical conditions did not have IHCPs on file.
The medications for a child with three diagnosed chronic health conditions were not properly labeled with the child's name. In addition the medications were not in their original containers with the prescription labels attached.
The program did not have parental permission to administer medications to a child diagnosed with three chronic medical conditions.
