It was observed that there were several ripped mats in the preschool room.
It was observed that the cushions on the children's couch and chair in the preschool room were ripped, closet doors were open and/or unlocked, plungers were accessible to children, and one chain on the window shade was not secured in the toddler room.
It was learned that one child's medication was expired, one child's medication was not in the original box and only had one Epipen, even though the IHCP calls for a 2nd Epipen if needed, and another child's prescription label had expired.
The fire inspection is expired.
