The assistant was unaware of the location of the first aid kit in case of an emergency.
The assistant allowed the (REDACTED) who to use the bathroom without supervision. After a few minutes, the assistant then went into the bathroom to assist the child.
A bottle of Febreze spray was accessible on top of the diapering station. A charging wire measuring longer than six inches was not secured and loose.
Supervision
The assistant did not document the attendance for today. The attendance records provided for review were not completed with documented time in and time out. They were documented stating if the child was present, absent, holiday or program closed.
Two toys were observed to have sitting water on them in the outdoor area.
The educator did not have documentation of the assistant's time in and time out when staffing the program.
Judgement
The licensor observed disinfectant wipes accessible to the children on the diaper table.
There is one enrolled child, who did not have an enrollment packet on the premises.

The Educator and assistant allowed the children in care to climb up a small metal step stool onto the toddler table. The children then were observed standing and walking across the table to sit in their seats for snack time.

The provider was not in compliance with staff ratios. There were 6 children present on the day of the visit. Four of the children were younger than 24 months and there was no assistant present.
During the visit the provider stated she wanted to use the living room space. The holes in the walls needed repair. There were items on the fireplace mantel within reach. A small table in the corner was accessible and tippable and the tread mill was accessible.

There were two toddler swings and a slide/climber in the outdoor space that did not have adequate fall zone protection under and around them.
The bedroom space needed to be painted and trimmed around floor and windows. The shelf near the bed was not secured. There were some cords hanging within reach. The gate to the living room area was not adequately secured.
Provider requested upgrade to 10 children. There was no evidence the fenced outdoor play area had adequate square footage for the upgrade. (750sf) Please submit a sketch with measurements.

There were hazards on the deck area path to the outdoor space such as broken shingles on side of the house and exposed nails on deck area. There was also miscellaneous wood and debris on the deck accessible.

Two children's records reviewed had not been updated annually. There were no current general permission forms indicating the provider had permission to take children off site.

Outdoor play
The provider stated that children were taken regularly off site for outdoor activities. There was no written plan or documentation available indicating how or where this occurred. Specific parental permission was not documented in each child's record for this to occur as required.
The educator is using her main bathroom due to construction. There was bleach and cleaning products accessible. There was also medication accessible

This renewal - full review visit inspection recorded no violations or advisories.
The licensor observed a red air compressor on the deck accessible to the children. The deck is used as an exit route for emergencies and as how the children enter the backyard play area.
