The heating board in the space in the basement was broken posing a risk to the children. In the upstairs kitchen there was “EZ fire spat” can accessible to the children in an unlocked cabinet and knives in the unlocked drawer were accessible to the children.
The second exit was being obstructed by a pack and play and other furniture.

The five children present record’s were reviewed. Two children needed an updated physical, two children needed lead testing and three children needed annual update forms.
