The educator was present with nine children when licensor arrived to the visit. The educator reported their assistant had stepped out quickly and was returning. The assistant left per security camera at 9:36 am and returned at 9:49 am. There were nine children present, five children are under the age of two years old.
The educator was outside on the porch with one child when licensor arrived to the visit. The remaining children were inside of the program. The educator was engaged in a tasks that unnecessarily diverted their attention from supervising the children. Four of the children were in the licensed bedroom in playpens with the door closed. The educator was not in sufficient proximity to the children in which they would have been able to intervene quickly when necessary. The educator was not providing direct supervision of the (REDACTED) in care who has been enrolled for less than six weeks.

Five of the children’s records were reviewed. One child needs proof of lead testing and an annual update form and one child needs an updated physical.
The educator is not documenting they are alternating exits on a monthly basis when conducting evacuation drills.
The educator did not have their assistant's training or medical documentation in their staff record.
The educator, assistant, and the children did not wash their hands with liquid soap and running water before or after handling/eating food. The educator did not wash the child’s hands or their hands after diapering the children.
In the outdoor space, there were over a dozen of deflated balloons with strings on the ground. On one of the stairs leading to the outdoor space, a section was disconnected and exposing nails.
Two of the A.C. units had cords which measure over six inches and were loose and not secured. In the kitchen, two drawers were not locked allowing access to a sharp knife, lighter, cleaning supplies and trash bags. On the changing stattion, there were various objects unrelated to diapering including a safety kit with screws.
