Current consent for administered medications
A review of three staff records indicated that all did not have evidence of immunization records.
It was observed in the SANBORN building's kitchen that there were snack food items not stored in a covered container.
The following was observed and accessible to children: 1) On the SMALL PLAYGROUND, a section of the wooden fence was missing a panel and had splintered edges (a different section was cited on 4/7/25). 2) On the wooden deck, there was a chipped and splintered piece with sharp edges. 3) There was chipped and flaking paint on the wooden train.
A review of four individual health care plans indicated the following: 1) One did not identify which program staff were designated and trained to administer a prescribed medication. 2) One was not fully completed and not signed by the parent. 3) One was expired.
It was observed in the PRE-K classroom that multiple children's water bottles were not labeled with the child's name for whom they are intended, and there were two sets of duplicate bottle designs.

During the visit, it was observed in PRESCHOOL 1 and 2 that the garbage containers were uncovered and contained discarded food items.

It was observed that on the playground fence gate, there were protruding bolts, a potential entanglement hazard.

A review of five children's records indicated the following: two did not have updated enrollment information and four did not have evidence of a current physical examination.

Upon a review of nine medications stored at the program, the following was identified: -One child's (REDACTED)

A review of the Background Record Check (BRC) Navigator system indicated the following: -One employee working in the program does not have a current and suitable BRC and was due for renewal in April 2021.
Upon a review of medication stored at the program, a child’s prescribed (REDACTED) was not in the original box. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.

Upon a review of five Children’s Records, five did not have a complete Individual Health Care Plan (IHCP). The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the childs health if the treatment is not administered. In addition, the plan must include the names of the educators in the program that have been trained in the child’s medical condition and the name of the individual/agency that trained those educators.
This renewal - monitoring visit inspection recorded no violations or advisories.

During the visit, it was observed that an Educator in PRESCHOOL 1 left the classroom to answer the door and meet a child being dropped off, this left the classroom out of ratio with 1 Educator to 17 children. The Program Director stated that children dropped off late are to report to the main office as they are available to bring the child to their respective classroom.
During the visit, it was observed in PRESCHOOL 2 that the Dramatic Play shelf was unsecured and tippable
During the visit, the following was observed and accessible to children: In CHILDREN'S BATHROOM 1 and 2 in the FAIR BUILDING, the floor air vent was rusted, and one was loose.
During the visit, it was observed in PRESCHOOL 1 that children's medical information was not privacy protected.
During the visit, first aid kits were reviewed and were found to be missing a CPR mouthguard.
It was observed that children's water bottles were not all labeled in PRESCHOOL 1 and 2.
A review of six IHCPs indicated that two were missing all required information such as which staff are designated and trained to administer the medication and what are potential side effects of the medication and potential consequences if medication is not given.
The following was observed and accessible to children: In the PreK classroom, the tall wooden dollhouse was not secured and tippable. In the Preschool 2 children's bathroom, there was a plunger and toilet brush located next to the toilet.
In the first children's bathroom, there was chipped paint located around the toilet.
A review of five individual health care plans indicated that three did not indicate which staff were trained and designated to administer medication in case of an emergency and one was expired.
It was observed that there was an open box of gloves in the first children's bathroom and in the Pre-K classroom.
