2024 evacuation log is complete. Educator has not started a log for 2025. Evacuation drills must be conducted with the children and documented every month.

Progress Reports
Five children's files were reviewed during the visit. Annual update forms were signed in September 2023 and January 2023. New Annual Update forms must be signed for children this month and one child had an annual update form that was last signed in 2022. Physical/immunization documentation is up to date.

This investigation inspection recorded no violations or advisories.
First aid kit is missing scissors and tweezers. Not all items could fit in the kit currently being used. Discussed the kit being more mobile and complete by placing items in a backpack. Discussed EpiPens going everywhere the children do, including outdoor play. EpiPens should be added to the mobile first aid kit. Emergency index cards with children's contact information are current.
Educator has her own parent handbook with policies such as medication administration, illness, pick up and drop off etc. Some EEC required information is missing from the handbook such as mandated reporting and safe sleep. Educator must review her handbook and add the missing information by comparing hers to the EEC parent handbook. Educator must email a copy of the completed handbook to the licensor for review.

Educator sends home daily notes to the parents but does not complete formal progress reports. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older).
Refrigerator temperature was unable to be verified. If the educator installs a small refrigerator in the basement, this must have a thermometer inside. Temperature must always be between 32 and 40 degrees.
Hot water temperature was 129 degrees. Temperature should not exceed 120 degrees. Discussed shutting off the hot water to the bathroom sink.
Discussed the educator staying on the same floor level as the children at all times. The educator will leave the children in the basement while she goes upstairs to the kitchen to prepare meals. The educator was given the option of licensing her kitchen in order to the bring the children up with her or creating a kitchen area in the daycare room with a small refrigerator, microwave, etc. so she will not have to leave the children to go to the first floor.

Allergy list and the location of a [REDACTED] not posted. Emergency phone list is posted on a wall by the door to the first floor with the fire evacuation plan. Child contact information is stored in the files, in the educator's cell phone, and in the first aid kit.
CPR/First Aid Certification
CPR and First aid certification expired on 2/20/21. The educator must take a recertification course ASAP and email the licensor a copy of the card or certificate. At this time, EEC is accepting online recertification due to COVID-19.
One child with an [REDACTED] and an [REDACTED] . [REDACTED] is stored in a file bin out of reach of children in the prescription box. No Individual Health Care Plan from the child's doctor nor Medication Administration consent forms signed by the parents are present. These forms must be completed and kept on file at the program.

Reviewed five children's files. Educator uses the Annual Update form and EEC enrollment packet. One child needs a new Annual Update form signed. Another child is missing a current [REDACTED]
Observed Shout spray accessible in the bathroom. Educator moved the bottle during the visit. Shed doors in the backyard were not all locked giving children access to the hazards they contain.
Peeling paint was observed outside around the door frame to the basement entrance.Educator must sign and return the Peeling Paint Compliance statement and email photographs of the peeling paint corrective actions to the licensor.
Ongoing Professional Development
First aid kit is missing non latex gloves, gauze pads, an instant cold pack, scissors, tweezers, and updated emergency contact information for the children currently in care.
Reviewed 22 hours of professional development training. Educator has not completed the required 12 Essential Strong Start trainings. Educator will email licensor the certificate when completed.
Front steps and walkway were not shoveled.

Educator does not practice toothbrushing with the children. Parents have not signed toothbrushing waivers. These waivers must be in children's files.
