Educator works with licensed assistant Becky Wersal. Educator could not locate her assistant’s file during the renewal visit.
On the Evacuation Log, Educator documented a fire drill every month this year, except for October. No fire drill was documented last month.
Licensor randomly reviewed 4 of the children's records. Of these, 1 was complete and up to date. The other 3 records all had out of date signatures in the enrollment packet, and 1 of these records also had no signature on pg.6, in the Basic Transport section.
Educator was not able to provide a copy of the program's Parent Handbook during the visit. Educator did not email the handbook to Licensor or upload it to her Document Library.

All of the required items were observed in the first aid kit, except for medical tape.
Educator appeared to be actively engaged with the children. The children appeared to be happy and well connected with the Educator. However, during the visit Licensor observed Educator's assistant speaking very abruptly and harshly to the children. Discussed with Educator that this needs to be addresses as it is a concern.
Licensor inspected all of the indoor, licensed space during the monitoring visit, as well as the outdoor playspace, and observed the following hazards: **DININGROOM** - no hazards ""LIVINGROOM** - outlets not covered on a power strip. **KITCHEN** - no hazards **PLAYROOM** - no hazards **BATHROOM** - hygiene products on sink, back of toilet, and in shower **OUTSIDE** - peeling paint on the deck outside the back door.
The first aid kit had out of date emergency contact information. All of the required items were observed, except band aids.
Licensor inspected all of the licensed, indoor space during the visit, as well as the children's outdoor playspace. The following hazards were observed: **DININGROOM - no hazards **LIVINGROOM - no hazards **KITCHEN - no hazards **PLAYROOM - no hazards **BATHROOM - clorox cleaning spray was hanging on the towel rack **OUTSIDE - chipping/peeling paint on the small deck in the back of the home.

Licensor reviewed 5 of the children's records. Of these, 2 were complete and up to date; the other 3 records had out of date physicals and out of date signatures on the enrollment packets. There were also 2 records that had out of date lead testing.
Educator has completed a fire drill every month. However, the date that the drill took place has not been entered for the past several months (since May).
All of the required items were observed in the first aid kit, except band-aids. The children's emergency contact forms were also out of date.
Educator does not have a record for her assistant.

Outdoor Playspace - Hazards
Educator utilizes a fenced in area in the backyard as the children's outdoor playscape. The paint around the trim on the back door is peeling and chipping off, as well as the paint on the steps to the deck.
Licensor did not observe any immediate health risks during the monitoring visit. All of the required items were observed in the first aid kit, except an instant cold pack. This item needs to be added to the kit. The children’s emergency contact forms also need to be updated. Tested the combination smoke and carbon monoxide detector in the hallway in the middle of the home and it worked appropriately.

Licensor reviewed 2 of the children's records during the virtual visit. One record was complete and up to date. The other record had out of date medical information.
Educator tracks the attendance in a record keeping calendar. She does not track the time of arrival or departure. Discussed that this information needs to be tracked daily for each child.
All of the required items were observed in the first aid kit, except for an instant cold pack and a thermometer.
Educator was missing October 2019 and November 2019 evacuation logs. All previous logs were missing time in which the evacuation was completed. Please ensure evacuations are completed monthly and recorded appropriately.
First Aid Kit is missing instant cold pack and emergency cards for children.
Water was tested and it was 130F degrees. Please ensure the water does not exceed 120F degrees.

Educator utilizes an assistant irregularly. No record is kept for assistant. Licensor discussed with Educator that she must keep the following on record for all assistants: health records, copies of licenses, certifications registrations held, including, but not limited to, motor vehicle operators license, documentation of orientation, training and professional development.
Six records were reviewed. Two children require progress reports. Please ensure these are completed and explain how progress reports will be maintained in the future.
Educator was missing attendance for this week. Please ensure attendance is completed daily with children's name, time of arrival and time of departure.

The steps leading to outdoor play space exceeds 3 steps and requires a gate. Please ensure a gate is available for these steps. There was also chipping paint on the deck and play structures outdoor that exceed 18 inches.
Educator did not have sufficient training hours. Please ensure 10 hours of training are completed each year.
