Children's Records
Licensor randomly reviewed 3 of the children's records. Of these, 2 were complete and up to date, but the 3rd had an out of date physical and immunizations.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace (fenced in area in the side yard). The only hazards were observed in the family room. There was an outlet near the fireplace that was not covered, and 2 shelving units near the window, which are both tipping hazards.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor space (fenced in area in side yard. The following hazards were observed: **DINING ROOM** - 5ft nutcracker is a tipping hazard **KITCHEN** - top drawer on peninsula has hazards such as scissors, sharpies, etc and the lock was not engaged, disinfecting wipes on counter, dog food on floor **FAMILY ROOM** - one outlet not covered **ACTIVITY ROOM** - no hazards **BATHROOM** - no hazards **BASEMENT: PLAYROOM** - no hazards **OUTSIDE** - no hazards

Educator's CPR/1st Aid certification was issued on 8/29/24. However, Educator informed Licensor that the entire course was done all online. Discussed that an in person skills portion needs to be completed per EEC policy. This needs to be done within the next week.
All of the required items were observed in the first aid kit. However, the children's emergency contact information needs to be updated.
There are multiple exits on the first floor and the basement level. Two exits on each floor were clearly marked and easily accessible. However, right now Educator utilizes the garage as the 2nd exit from the basement level. There were multiple hazards observed in garage, including wd40, shelves are tipping hazard, snow blower, etc.
Educator's license was in a folder on the wall. Discussed that the license has to be posted somewhere visible in the program.
Stairways
There was no gate on the stairs leading from the first to the second floor.

All of the required items were observed in the first aid kit. However, the children’s emergency contact information was out of date.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was in the Activity Room on the first floor. There was a shelving unit with holiday village houses on it, which is a tipping hazard.
Licensor was able to inspect all of the licensed space in the home, as well as the outdoor playspace. The following hazards were observed: ** DINING ROOM - no hazards ** KITCHEN - long drawer with scissors, sharpies, lighters, etc, inside, and bottom drawer next to it with batteries and other hazards inside ** FAMILY ROOM - no hazards ** ACTIVITY ROOM - no hazards ** 1st FLOOR BATHROOM - trash does not have a cover ** BASEMENT: PLAYROOM - no hazards ** BASEMENT BATHROOM - no hazards ** OUTSIDE - no hazards
