General: Based on observation, the surveyor found the following: 1. There were cigarette butts and paper debris on the ground in the parking area leading up to the front entrance of the childcare center; therefore, this area was not kept clean. 2. In the Preschool Classroom, there was a broken air vent in the floor next to the step leading into the Two Year Old Classroom; therefore, this was not kept in good repair. 3. There were cob webs on the wall postings as well as dirt and debris on the floor of the Preschool Classroom; therefore, these areas were not kept clean. 4. In the Blue Toddler Classroom there was dirt and debris on the floor as well a rug and the diaper changing pad. There were also what appeared to be rodent droppings on the floor next to a set of cubbies as well as on a red pillow on the floor of the classroom. Therefore, the classroom was not kept clean. Staff-in-charge stated that this room had not been used for a few months due to staffing. 5. On the windowsill of the Blue Toddler Classroom there was debris and a patch of paint which had peeled away from the sill; therefore, this area was not kept clean and in good repair. 6. Dirt and debris on the floor and diaper changing pad in the Green Classroom, as well as dead insects in the sink in this classroom. Staff-in-charge stated that this classroom had not been used in a few months due to staffing shortage. 7. In the downstairs restroom located behind the kitchen, there was a broken toilet paper dispenser on the wall and a roll of toilet paper placed on the window sill, and a yellow substance in the unflushed toilet; therefore, this area was not kept clean and in good repair. 8. A blue oval rug in the Two Year Old Classroom was covered in debris and dark spots; therefore, this rug was not kept clean. 9. There was a gap behind the door lock on the door leading to the screened play area from the Two Year Old Classroom; therefore, this door lock was not in good repair. 10. In the screened play area located next to the Two Year Old Classroom there were multiple broken pieces of the wooden lattice covering the screened windows with jagged edges; therefore, this lattice work was not kept in good repair. 11. There was paper and plastic debris on the floor of the screened play area, as well as a stainless steel garbage can that was overflowing with garbage; therefore, the screened play area was not kept clean. 12. In the upstairs restroom next to the Science Room a portion of the rubber baseboard was peeled away from the wall, the trash can was full and emitting a foul smell, and the soap dispenser was detached from the wall and not fastened securely; therefore, this restroom was not kept clean and in good repair. 13. The ceiling of the upstairs Homework Room had three (3) water stains as well as one (1) black stain; therefore, the ceiling was not kept in good repair. 14. In the upstairs Recreation Room, there was a portion of the rubber baseboard peeling away from the wall and a layer of dust on the ceiling fan. Therefore, this area was not kept clean and in good repair. 15. There were dead insects on the floor, in the bathtub, in the double sinks and on the vanity of the large upstairs restroom next to the Recreation Room, as well as cobwebs in the corners of the room and doorway. Therefore, this restroom was not kept clean. 16. There were dead insects on window sills and floors of the upstairs Computer Room and Homework Room, as well as dirt and debris on the floors; therefore, these areas were not kept clean. 17. General: Based on observation, the surveyor found the following: 1. A crib mattress wrapped in clear plastic that was turned upright and stacked on top of what appeared to be a play size wooden stroller located in front of a closet opening in the Green Classroom. 2. A small sunbeam fan placed on a shelf in the Toddler Classroom (Blue Room) with the cord dangling down. 3. A black bucket that had “statesman” written on the front of the bucket and contained what appeared to be seed located in the floor in the Toddler Classroom (Blue Room). Staff-in- charge stated that she unsure what was inside the bucket. 4. Multiple shopping bags placed in an unlocked cabinet underneath the diaper changing pad table in the Toddler Classroom (Blue Room). The cabinet was observed to have what appeared to be a broken child safety lock. 5. A hammer, flat screwdriver, pliers, and nails in a clear container in an unlocked drawer in a cabinet in the Kitchen located on the end beside the Toddler Classroom (Blue Room). The Infant Classroom (Purple Room) was observed to not have a door separating the classroom from the Kitchen. The surveyor observed the Toddler, Orange, Green, and Pink Classrooms and restroom in an area that children would walk through the Kitchen to access the classrooms. Therefore, the items were observed to be accessible to the children. 6. Knives placed inside an unlocked drawer in a cabinet in the Kitchen located on the end beside the Infant Classroom (Purple Room). The Infant Classroom (Purple Room) was observed to not have a door separating the classroom from the Kitchen. The surveyor observed the Toddler, Orange, Green, and Pink Classrooms and restroom in an area that children would walk through the Kitchen to access the classrooms. Therefore, the items were observed to be accessible to the children. 7. A wooden/plywood square board placed behind a crib in the Infant Classroom (Purple Room) that contained a chipped/splintered end. 8. A little trikes box placed on a railing and leaned against a shelf in the Play Room located beside the Infant Classroom (Purple Room). 9. Based upon interview, the provider suggested that the odor was caused by old pipes in the building and would diminish when water flushed through the plumbing system. The provider stated that a head plumber and the health department had inspected the premises and determined it to be safe; however, continually smelling the pungent odor is a detriment to the children's comfort while in the center.

The staff person in the classroom stated that the children ranged from two (2) months of age to three (3) years of age and that the three-year-old child did not feel well and had been in the Infant classroom all day.
