General: Based on observation, the surveyor attempted a visit to the provider's home on December 9, 2025. The surveyor tried knocking on the front door multiple times with no one answering. General: Based on review of documentation, the surveyor discovered the following: 1. General: Based on observation, the surveyor discovered that there were three (3) adult english bulldogs in the provider's home. Two (2) of the english bulldogs were in a cage located in the living room. The surveyor observed one (1) english bulldog lying on the couch. The children were present in the living room with the dogs. During interview, the surveyor learned that the english bulldog on the couch had two (2) puppies lying on the couch with her. General: Based on observation and interview, the surveyor found the provider was not open during regular operating hours on 10/02/19 and had not notified Cabinet designees about the closure. Therefore, the surveyor was not able to enter the family child care home during operating hours to conduct the survey. Staff (DOH: 11/15/93) had completed Pediatric Abusive Head Trauma training on 09/30/13. A review of the staff's file and a review of ECE-TRIS found no additional PAHT training; therefore, the PAHT training is not being completed every five (5) years. General: Based on observation at 10:10 AM on 08/19/15, the provider was caring for five (5) children ages two (2) months-old to four (4) years-old. The two (2) month-old infant was sleeping in a Fisher Price vibrating hammock rather than a crib as required by regulations. During an interview with the provider, she stated that the child was not enrolled; however, she was providing care for the child. On 08/19/15, the provider stated that she had not purchased a crib.

General: Based on interview with the provider, she stated that a sibling group of three (3) children were enrolled on 07/06/15; however, enrollment documentation was not completed and available for review. Therefore, the following required documentation was incomplete on each of the three (3) children: a) Information about the child, which includes, the child's name, date of birth, and address. b) Contact information to enable the provider to contact the child's: parent at home or place of employement, family physician, and preferred hospital. c) The name of the of the person who is designated in writing to pick-up the child. d) The child's general health status and medical history. e) The name and phone number of each person to be contacted in an emergency situation. f) Authorization by the parent for the provider to seek emergency medical care for the child in the parent's absence.
General: Based on review of documentation, the provider did not provide documentation that evidenced that she had coordinated at least one (1) annual activity involving parental or family participation. During an interview, the provider stated that she had hosted parent/family involvement activities such as Christmas and Easter parties; however, she stated that she did not retain written documentation of those activities. The plastic bags were accessible and within reach of the children. 3) One (1) roll of plastic trash bags were located inside of a white cabinet above the toilet. The cabinet was unlocked and the trash bags were accessible and within reach of the children.

General: Based on observation, an eleven (11) month-old infant (DOE 07/06/15) was sleeping on the provider's couch during the DRCC inspection. During an interview, the provider stated that she did not have a crib and had not provided care for an infant recently. The provider stated that the child would be turning one (1) year-old in one (1) week from the date of the inspection; however, the provider did not have completed enrollment information on the child to verify the date of birth.
General: Based on review of documentation, a weekly menu was not posted as required, nor provided for review during the DRCC inspection on 07/08/15. During an interview, the provider stated that she could not locate the menu.

General: Based on observation during a tour of the certified family child care home, the surveyor found containers of Neosporin and Orajel in an unlocked sink drawer and a bottle of hair remover on the counter top of the sink located in the restroom that is used by the children. The medication was not in a locked container or in a locked area.
