General: Based on review of documentation, and KY-TRIS the surveyor discovered that a staff member (DOH: 09/02/24) completed fourteen and one half (14 1/2) of the required fifteen (15) hours of cabinet-approved early care and education training completed between 09/02/24 and 09/02/25.

General: Based on review of documentation, the surveyor found that the staff’s (DOH:07/13/23) file that contained documentation from KARES that the staff person was eligible for hire on 07/07/23. On the day of visit, the surveyor printed the child-care center’s KARES report and found that the staff person was not listed under the current staff list; however, the staff was listed as "separated". Therefore, the child-care center failed to update the staff person’s employment status. During interview with staff-in-charge, she stated that the staff person had been rehired as a substitute staff person on 07/19/24 and the KARES report had not been updated. General: Based on interview, the surveyor found that a staff member inappropriately handled a four-year-old child. On 07/22/24, a staff member was caring for children in the Preschool Classroom when she picked up a four-year-old child by the wrist, then released the child to fall back down onto the floor. During interview with multiple staff, the surveyor learned that the child complained that her arm was hurting and needed an icepack. The surveyor reviewed video footage and observed the staff member person to pick up the four- year-old child by the wrist in an aggressive manner, then released the child onto the floor; therefore, the health, safety and comfort of the child was compromised.
General: Based on interview and review of documentation, the surveyor found The Division of Fire Prevention's Local Inspection Summary was dated 08/10/17. Staff reported they have had no contact with the Office of the State Fire Marshal or designee since 2017. Staff advised they will make contact and request a current inspection. General: Based on review of documentation and review of the ECE-TRIS Required Training Hours Report the surveyor discovered the following: 1. For the timeframe of 7/1/21 thru 6/30/22, the surveyor discovered that one (1) staff (DOH: 8/20/07) completed fourteen (14) hours of the fifteen (15) required hours of cabinet- approved early care and education training. 2. For the timeframe of 7/1/21 thru 6/30/22, the surveyor discovered that one (1) staff (DOH: 8/6/19) completed eleven (11) hours of the fifteen (15) required hours of cabinet- approved early care and education training. 3. For the timeframe of 7/1/21 thru 6/30/22, the surveyor discovered that one (1) staff (DOH: 9/2/16) completed six (6) hours of the fifteen (15) required hours of cabinet-approved early care and education training. Therefore, the child-care center failed to ensure that staff completed their annual training as required. Staff-in-charge stated after staff completed some trainings, staff learned that some of the training did not count towards their fifteen (15) required training hours. General: Based on observation, the surveyor found that the childcare center had renovated the Preschool 1 room by adding a non-permanent wall, to create a space that enables the one (1) year olds and two (2) year olds to be separate. General: Based on observation and interview, the surveyor found the following: 1. Two (2) infant children had a pacifier attached with a clip while asleep in their cribs. The clip provides a strangulation risk while the infants are asleep. 2. One (1) infant child sleeping in a crib was observed with a bib in their hand. The bib can cause a choking or strangulation hazard while the infant is asleep. 3. The Threes and Fours classroom, during naptime, were not properly lit, i.e., areas where children were napping were not visible. During interview with staff in charge, she stated that she was not aware that pacifiers could not have the clip attachments. She stated that the infant was wearing the bib when laid down to sleep and probably "ripped" it off. General: Based on review of documentation, the following was found: 1. A staff file (DOH: 6/1/17) presented for review did not contain a negative tuberculin skin test result. 2. A staff file (DOH: 9/12/16) presented for review contain a negative tuberculin skin test result that had been interpretted on 7/2/15; therefore, the staff member had not had a negative tuberculin skin test within the past two (2) years.
General: Based on observation, review of documentation, and interview, the surveyor observed staff in the Infant Classroom take a child directly from eating a snack to the diaper changing area to change the child's diaper without first washing the child's hands. The surveyor observed the correct handwashing procedures were posted in the classroom next to the sink. Staff reported they were aware of the proper handwashing procedures and when children's hands are supposed to be washed.
General: Based on observation and interview, the surveyor found two (2) soiled bibs on the floor in the Infant Classroom underneath the leg of the infant swing. The soiled bibs had not been stored temporarily in a covered container until they could be washed. Staff stated the bibs were from that morning. First Aid/Medication Not In Compliance
General: Based on review of documentation and interview, the following was found: 1. One (1) employee file (DOH: 08/20/2007) presented for review did not contain an up-to-date annual professional development plan. The last professional development plan in the file was dated for 09/28/2015. Upon interview, staff acknowledged that the employee's professional development plan was over due for an update. 2. One (1) staff file (DOH: 09/12/16) did not contain evidence of educational qualification as required by regulations. Staff in charge reported that they had not yet obtained record of education; however, they new that the staff member does meet the education requirements. 3. One (1) staff file (DOH: 07/08/16) contained record of a negative tuberculin skin test; however, the surveyor could not determine if the tuberculin skin test was completed within the past two (2) years or not as the record did not contain a date that the skin test was performed or interpreted. General: Based on observation, the surveyor found that the First Aid Kit did not contain a cold pack.
General: Based on review of documentation, the surveyor found that the file for a staff (DOH: 07/01/15) did not contain a criminal records background check or documentation to verify that the criminal background check had been submitted. Through interview, the surveyor learned that the staff person has not been alone with the children.
General: Based on review of documentation and interview with staff, the surveyor found that the copies of negative tuberculin test results contained in two (2) staff’s (DOH: 02/22/02 and 09/05/14) files were no longer current as of 10/01/14.

General: Based on observation, the child-care center did not have an individual non-tiered crib for each infant present at the time of the inspection visit as there were five (5) infants present in the classroom but only four (4) non-tiered individual cribs. Based on interview with child-care center staff, the oldest infant present in the classroom was ten months old.
General: Based on observation during a tour of the child care center, the surveyor found the following: 1. The surveyor found that a portable heater, located in the Infant and Toddler Room, had been placed on a shelf that is approximately two and a half (2 1/2) feet high. The heater was within reach of a toddler-age child. 2. The surveyor found that the temperature in the Infant and Toddler Room was sixty (60) degrees Fahrenheit. (3) Rest time shall include adequate space specified by the child’s age as follows: (a) For an infant: 1. A firm crib mattress in good repair with a clean tight-fitted sheet that shall be changed: a. Weekly; or b. Immediately if it is soiled or wet;
General: Based on observation, the surveyor found the following: 1. The children from the one- and two-year-old children's class were observed to be playing in the indoor gross motor area that has a poured concrete foundation and is absent a protective surface. Three (3) of the children were observed to be pedaling riding toys and running across the concrete floor; however, they were not wearing shoes. 2. The surveyor found that a toilet brush and holder were beside the toilet located in the restroom that is used by the preschool and two-year-old children.
General: Based on observation during a tour of the child care center, the surveyor found the following: 1. Empty plastic bags were observed to be in unlocked cabinets located under the sinks in the Preschool Room, School-age Room and Two-year-olds classroom. 2. A staff’s bag containing a billfold and another staff’s purse were observed to have been placed on the floor next to the sink located in the two-year-olds classroom. The items were accessible to the children. 3. A container of Goof Off Wipes was observed to have been placed on a shelf located by the door in the School-age Room. The wipes were accessible to the children.
General: Based on observation, the surveyor found that a bottle had been placed on the top of a shelf located in the Infant Room. The bottle was not in use and was not labeled or covered.

General: Based on review of documentation, the surveyor found that the Emergency Preparedness Plan was dated 08/12/13. Through interview with staff, the surveyor learned that there was no documentation to verify that the plan had been reviewed or revised within the past year.
General: Based on observation during a tour of the Infant Room, the surveyor found that the children's first and last names had been posted and were visible to others.
General: Based on review of documentation and interview with staff, the surveyor found that three (3) staffs (DOH: 08/20/07, 03/15/09 and 09/05/14) annual written performance evaluations had not been completed. General: Based on observation during a tour of the child care center, the surveyor found the following: 1. A toilet brush and holder had been stored beside the restroom door located near the kitchen. Through interview, a staff person confirmed that the restroom is sometimes used by the children; thus, the toilet brush and holder were accessible. 2. The surveyor found that a telephone cord, located in the Infant and Toddler Room, was dangling down within reach of the children. 3. Paint, pliers and screwdrivers were observed to have been stored in an unlocked furnace closet adjacent to the indoor gross motor area. The items were accessible to the children.
General: Based on review of documentation provided for three (3) persons identified as qualified substitute staff, the surveyor found the following: 1. A staff’s (DOH: 07/01/15) file did not contain a criminal records background check or documentation verifying that it had been submitted. 2. Review of ECE-TRIS and documentation contained in a staff’s (DOH: 05/27/15) file did not verify that the staff had completed six (6) hours of cabinet-approved orientation training. 3. A staff’s (DOH: 09/05/14) file contained a copy of a negative tuberculin result that was no longer current as of 10/01/14.
General: Based on observation, the surveyor found a thick layer of dust on the air vents located in the indoor gross motor play area.
General: Based on observation during a tour of the School-age Room, the surveyor found that there was no thermometer in the refrigerator; therefore, the interior temperature could not be determined.
General: Based on observation during a tour of the kitchen, the surveyor found that the interior top, sides and bottom of the oven contained dark colored stains and baked-on food particles.
General: Based on review of documentation, the surveyor found that a child’s (DOE: 11/25/14) file contained an immunization certificate that was no longer current as of 06/09/15.
General: Based on review of documentation and interview with staff, the surveyor found that four (4) staff (DOH: 02/22/02, 08/20/07, 03/15/09 and 09/05/14) did not have a current written annual plan for professional development.
General: Based on review of documentation, the surveyor was unable to verify that an earthquake or tornado drill had been conducted during the first quarter of 2015.
General: Based on review of documentation, the surveyor found the following: 1. The surveyor found that the file for a staff (DOH: 10/20/13) contained a criminal records check (CRC) dated 09/05/14. Through interview, the surveyor learned that the staff has worked past the ninety (90) day probationary period before a CRC was received. Interview confirmed that this staff has been left alone with the children prior to the center receiving the results of the CRC. 2. The surveyor found that the child care center had obtained a staff’s (DOH: 09/03/14) criminal background check from the state of Tennessee; however, the center had not submitted for or obtained a CRC from the state of Kentucky. Through interview, the surveyor learned that this staff has not worked alone with children.
General: Based on review of documentation the surveyor found the following: 1. Ten (10) of the thirteen (13) children’s files reviewed did not have information to enable a person in charge to identify and contact the child’s preferred hospital. 2. Two (2) (DOE: 12/24/12 and 02/07/13) of the thirteen (13) children’s files were reviewed did not have information to enable a person in charge to identify and contact the child’s physician. 3. The surveyor found that a child’s (DOE: 08/09/13) file did not contain medical information.
General: Based on observation, the surveyor found a bottle of generic pain reliever on a shelf located in the one- and two-year-old children's classroom. The medication was not stored in a separate and locked place. Outdoor Play Area 675 - Fence Requirement Not Applicable 922 KAR 2:120. Section 4. Premises Requirements. During record review, it was learned that a staff member's (DOH: 10/20/13) file contained a criminal records background check dated 09/05/14. It was confirmed through interview that this staff has been left alone with children before the results of a criminal records check were received.
General: Based on review of documentation, the surveyor found that a staff's (DOH: 09/05/14) file did not contain a copy of a negative tuberculin result or a statement from a health care professional that the individual is free of active tuberculosis.
General: Based on observation during a tour of the child care center, the surveyor found that three (3) one-year old children were being provided care with two-year old children for longer than one (1) hour. A review of documentation contained in the children's files revealed that transition plans had not been completed. Through interview with a staff, the surveyor learned that the children are combined for the entirety of the day.
General: Based on observation, the surveyor found the following: 1. A thick coating of dust was observed on the air vents located on the walls by the child care center's front entry door and outside of the Infant Room. 2. The Preschool Room door was in disrepair making it difficult to open and shut.
General: Based on observation and interview during a tour of the child care center, the surveyor learned that hot water was not available due to a leaking sink located in the School- age Room.
General: Based on observation during a tour of the Infant Room, the surveyor found that three (3) infant children were in attendance; however, only two (2) cribs were available to meet their needs.
General: Based on observation during a tour of the child care center, the surveyor found that the refrigerator located in the School-age Room did not have a thermometer. Additionally, the surveyor was unable to verify the temperature of the refrigerator located in the two-year-olds class as the thermometer was frozen within the ice of the freezer.
General: Based on review of documentation, the surveyor found that the file for a child (DOE: 03/10/13) contained an immunization certificate without an expiration date; therefore, the child’s immunization status could not be verified.
General: Based on review of documentation, the surveyor was unable to verify that a fire drill had been conducted during May 2014.

General: Based on observation, a toddler was sleeping on a pillow on the floor instead of on an individual mat or cot.
General: Based on observation, a staff person left the infant/toddler classroom to wash her hands after wiping a child's, nose leaving another staff alone with seven (7) infant/toddler children with the youngest being nine months old. The staff person was gone from the room for one to two (1-2) minutes.
General: Based on interview, the director stated that she did not have a copy and had not provided staff a copy of the most current regulations.
General: Based on observation, a nine-month-old child was sleeping in a bouncy seat on 11/12/13 during nap time.
General: Based on observation, there were three (3) infants in care who were sleeping on 11/12/13. There were only two (2) cribs available for the children. Therefore, an individual crib was not provided for each infant in care.
General: Based on review of documentation, the following was found: 1. The files presented for review for children (DOE: 06/14/13 and 8/19/13) did not contain the name of the preferred hospital, the family physician nor contact information that would enable a person in charge to contact the hospital and physician. 2. The files presented for review for children (DOE: 08/08/12, 7/30/12, 10/08/12, 8/7/12, 3/20/13, and 1/7/13) did not contain contact information that would enable a person in charge to contact the preferred hospital.
General: Based on review of documentation, the file for a staff member (date of hire - 6/4/12) did not include a high school diploma, GED or Commonwealth Child Care Credential.
Based on observation during a tour of the child care center, the surveyor found that the handwashing procedure was not posted at the infant or toddler diaper changing areas.
