General: Based on observation, the following was found: 1.) Brooms and dust pans were stored on the floor or hanging low on the wall, accessible to the children in care in Rooms One and Five. This method of storing promotes the spread of germs and bacteria. 2.) Storage cabinets were not locked in Rooms Four, Six and Seven. The cabinets contained shaving cream, plastic bags, hard candy and caramel apple on the stick. These items were accessible to the children in care. 3.) The sink that children use for handwashing was not clean in Room Two. General: Based on review of documentation, it was found that personel files only contained verification that fingerprints had been obtained for staff that have completed the National Background Check. The document that provides an eligible for employment date was not on file for those staff members who have completed the process. Although a review of KARES shows the eligible date of hire, the documentation was not on file at the facility. Programming In Compliance Premises Not In Compliance

922 KAR 2:110. Section 5. Staff Requirements.

General: Based on Review of Documentation, this regulatory requirement was not met. One (1) staff person did not have evidence of a negative TB skin test or physician's statement documenting he/she was free from tuberculosis prior to employment. The staff person was hired on 8/1/16; however, did not have a TB skin test until 9/14/16.
