The surveyor was able to verify via KARES portal that this individual had completed background checks on 10/3/19, and had submitted fingerprints on 10/9/19. General: Based on Review of Documentation, a review of children's files revealed two (2) children enrolled on 8-18-15 had immunizations on file that were expired. Based on review of documentation, a review of children's files revealed a child enrolled on 8-19-15 had immunizations on file that did not have an expiration date listed. The director and site supervisor acknowledged this and stated they had not yet recieved updated immunizations for the children enrolled on 8-18-15, and stated they could not locate an expiration date on the immunizations for the child enrolled on 8-19-15.

General: Based on Review of Documentation, the center did not have an up to date emergency preparedness plan on file. The plan reviewed was dated for 10/30/14. Based on interview with the site supervisor, she stated the center had completed un updated plan, and was not sure why a copy was not located in the file at the center.
