General: Based on observation and interview, the following was found: Infant room – In the refrigerator there were three (3) uncovered sippy cups. Toddler Room – In the refrigerator there were eight (8) uncovered sippy cups. An interview with staff found the sippy cups are washed after the children eat breakfast and are then filled with milk for lunch and stored in the refrigerator until lunch time. Although the cups are washed after each use this method of storage is unsanitary due to the drinking spouts on the cups being exposed to any contaminates there may be inside of the refrigerator. General: Based on interview and a review of documentation, it was found that all but two children enrolled in the facility receive funds from the Child Care Assistance Program. A review of daily attendance records for the week of 05/06/24 found there was no time of arrival or departure documented for the children in attendance. However, the signature of the child’s parent or guardian had already signed verifying all the times of arrival and departure were accurate even though the week was not finished, and the times were not noted. It should be noted the program has an electronic sign in/sign out system where parents clock in electronically the time of arrival and departure. An interview with staff found that the times of arrival and departure are then pulled from the electronic sign in/sign out and recorded on the form used for Child Care Assistance. General: Based on observation, staff confirmed that on 11/12/2020, the morning temperature in the building was 62 degrees. By the end of the day the temperature went to 69 degrees. The staff kept the children in coats until lunch time to keep them warm. The parents were informed of the situation at the end of the day on 11/12/2020. The center closed on 11/13/2020 and 11/23/2020, for heating repairs.

The staff person in the Early Head Start classroom stated that she had "ran out" of whole milk and had to serve one (1) percent milk. General: Based on Observation, upon arrival into the Early Head Start classroom at approximately 9:52 a.m., the surveyor heard the radio playing. The song that was heard playing on the radio was "Hells Bells" by AC/DC. The song played almost through its entirety before the staff person turned the radio off.
