General: Based on interview and review of documentation, the following was found: 1) There were four (4) staff members listed under the facility license number in the KARES system who no longer work at the facility. The staff member in charge indicated these staff have transferred to one of their other locations. 2) There were two (2) staff members listed under the facility license number who are no longer employed. 3) There was one (1) staff member working at the facility who was not listed under the facility's license number in KARES. The staff member in charge indicated this staff member transferred from one of their other locations in August 2023. The employment status of each staff member was not maintained in the KARES background check system. The KARES background check system was reviewed and confirmed these findings. 922 KAR 2:120 · 922 KAR 2:120. Section 2. Child Care Services.

General: Based on Interview, it was revealed that a staff person told a four (4) year old child to bite a three (3) year old child back after he had been bitten on the arm on 4/24/13. A statement written by the staff person was presented for review during the investigation. The statement read, "I went to get (the biter) and brought him over to the sink area where the child who had been bitten was. I instructed the child to bite (the biter) back, trying to impress upon how biting hurts." A witness to the incident reported that the staff person told the child who had gotten bit, to bite the child back has hard as he could. Based on written statements and staff interview it was confirmed that the staff person involved in this incident failed to provide and promote the children's emotional and social well being in a positive manner and according to each child's developmental level.
