National Background Check Service found two (2) staff did not complete background checks prior to becoming a child-care staff member. There was a brown rectangular table that at numerous areas of the laminate that had come off of the table top. There was a low round table that had tape residue with an accumulation of dirt and grime and dried paint on the table top.

The parent reported this observation to assistant director who was in charge at the time. The parent was informed the director would be made aware of the incident and the staff person would be removed from the classroom. Further interviews found that the director was notified by the assistant director later that afternoon. It was also found that the director spoke on the phone with the parent later that evening and the parent was informed the incident would be reported to the facility's Day Care Board.
