General: Based on observation, the center failed to maintain requirements based on state regulations. Upon observation, it was revealed the bathroom sink's water did not reach minimum required temperature for hot water. The water was cold to the touch and never became warm enough to meet regulation based on the use of a thermometer. In the exit interview with the director, it was revealed that the hot water valve beneath the sink had been turned off.
