Based on the program’s self-report, staff member A.A. did not follow required procedures for documenting an injury that occurred while the child was in care. The director reported that A.A. was present during the incident and failed to complete the injury report at the time of the event. No written injury report was prepared or provided to the parent at pick-up, despite the child receiving on-site medical treatment that day. The director further reported that an injury report dated 05/29/2026 was later completed by A.A.; however, this report was not provided to the family.
