At the time of inspection there was no running water in the bathroom sink in room 2.

In room 10 the diaper pail did not have a tightly covered lid that could control the odors.
At the time of inspection rooms 4, 5 and 6 had foam unit blocks, not the required wooden unit blocks.
In room 4 there was paper towels stored open under the sink, not sanitary. Room 5 had paper towels on the back of the sink, not on a dispenser and toilet paper stored open in the bathroom. Room 10 had toilet paper on top of the dispenser, none in the dispenser.
4 staff files were randomly checked for drug tests based on a complaint, 2 staff were missing documentation of a drug test. Send documentation to consultant.
Room 9 with 3/4yr olds did not have the proper number of caregivers caring for the number of children in the classroom. They had 1 caregivers with 11 children and the required ratio for that age group is 10:1. Room 2 with infant and toddlers did not have the proper number of caregivers caring for the number of children in the classroom. They had 1 caregivers with 5 children and the required ratio for that age group is 4:1.
At consultant's arrival, a 4month old infant was seen sleeping in the swing.

On 10/1 the Bears room was out of maximum group size for 30month old children with 24 children ages 30months to 4yrs old.
On 10/1 the Bears room was out of ratio with 24 children ages 30months to 4yrs old with 3 staff. At the time of todays inspection the Zebras room was out of ratio with 7 children ages 12mon. to 18mon. and 1 staff.
At the time of inspection, the toddler and 2's rooms were served Sunflower nutbutter on spoons for snack which is a choking hazard.
Hazardous items and items labeled Keep out of Reach of children were found accessible in the following areas: Room 1 had Dawn dish soap under the sink behind a child deterrent. Room 2 had Swiffer cloths and Miracle cleanser sponge under the sink behind a child deterrent. Room 4 had a bucket of sunscreen and bucket of diaper cream under the sink behind a child deterrent. Room 6 had a teacher's purse, Lysol wipes, Febreeze and windex on low shelf in an unlocked closet. Rooms 5, 7, 8, and 9 has paint peeling on walls.
At the time of inspection an art easel was not seen room 2.
Room 5 had open diapers being stored on the changing table. Room 9 had pull up stored on top of the paper towel dispenser. Room 10 the caregiver place diapers on the floor when changing children's diapers.
Room 10 had a vacuum in one of the bathrooms.
In room 1 there was a container of infant puffs, 1 bag of yougurt bites, and 1 container of cereal not dated.
Alphabet rug in room 6 is not secured to the floor.

Kangaroos classroom sink is slow to drain.
In the Meerkats room there was a baggie of meat and cheeze-its not dated. In the Kitchen there was a box of chex cereal, bag of granola and 2 boxes of pasta not dated.
Meerkats had paper towels open on a shelf. Kangaroos have open paper towels and bowls under the sink. Dolphins had paper towels on the table open not on a dispenser. Giraffes have paper towels open on the shelf, not on a dispenser.
Penguins had red and blue mats that are torn, not sanitizable. Kangaroos had a blue climber that is torn, not sanitizable. Dolphins have torn red steps, not sanitizable.
The Kangaroos, Dolphins, Tigers, Cheetas, and the Bears have paint peeling on the walls.

Zebras had a toy in the bathroom. Dolphine had a dust pan/broom and swiffer in the bathroom. Tigers had art work hanging in the bathroom, not sanitizable. Giraffes had art in the bathrooms, a plunger, a toilet cap missing and an open bucket of sunscreen in the bathroom.

Zebras classroom sink needs to be resealed to the wall.
Zebras and Penguins have open diapers under the sink, not sanitary. Sealions have open diapers in the bathroom, not sanitary. Bears have open pull ups in the bathroom, not sanitary.
There was no water at the bathroom sinks in the Zebras, Penguins, and one of the bathrooms in the Tigers room. The Sealions classroom sink did not reach 100 degrees F.
