During visit consultant observed the Twos class out of ratio with 13 children ages 26 months to 30 months with 2 qualified caregivers. Twos ratio is 5:1.
During visit Young Twos room was out of Group size with 13 children ages 26 months to 30 months old. Max group size for a class with 2 year olds is 10.
At time of visit, kitchen missing A person with BCC approved food service training will be present whenever meals or snacks are served.
Dry food goods and chemicals/cleaners must be stored seperately- Bleach bottles were stored on shelf above opened bin containing sugar in the kitchen.
Toddler restroom being used for storage- contained easel, and kitchen set.
At time of visit, all classrooms missing approved first aid directives in each classroom.
At time of arrival consultant observed the school age classroom did not have the proper number of qualified caregivers directly caring for the number of children in the classrooms. They had 0 qualified caregiver with 1 nine year old child and the required ratio for that age group is 15:1.
Items labeled "Keep out of reach of children" must be inaccessible at all times. Twos classroom- Restroom cleaner spray on the changing table. Toddler 2 classroom- restroom cleaner on top of changing table- diapers creams and ointments accessible in unlocked cabinet below. Kitchen and side door both unlocked and accessible to children- Bleach, restroom cleaners, window cleaners, disinfectants all within reach of children.
At time of arrival consultant observed the school age classroom was not supervised by qualified caregivers as 0 staff present with 1 nine year old boy. During visit, consultant observed toddler classroom with 5 toddlers not being supervised by 1 caregiver sleeping in chair.
Center has multiple broken and missing screens in windows.
Older twos room had paper towel sitting on counter not on a dispenser and toilet paper sitting on ledge in the restroom not on a dispenser. School age classroom had toilet paper sitting out on classroom sink.
Food in the kitchen refrigerator missing date of opening on cheese, scrambled eggs and mash potatoes. All food must be dated with preparation and what it is.

Caregiver left the PreK classroom with 12 children unsupervised to answer the door for consultants entry.
•During visit consultant observed the PreK class out of ratio with 12 children ages 3 yrs old to 9 yrs old with 1 qualified caregiver. Ratio for 3 yr old to 5 yr old is 10:1. Caregiver left the PreK classroom with 12 children unsupervised to answer the door for consultants entry. ratio at that time was 12-0. Twos room, had ratio of 12:2 with 2 toddlers within age range. Ratio for 2 year olds is 5:1.
Items labeled "Keep out of reach of children" were accessible to children in the following rooms; Toddler room- Unlocked changing table contained- Triple paste, Desistin, Ecolab Lime Away gallon jug. Two's classroom- unlocked changing table contained Desitin, Aquaphor, Vaseline. Unlocked cabinet (key in lock) contained Caregiver purse within reach of children.
Unlocked door with key in lock to kitchen/storage contained various cleaners including floor cleaner, glass and multi-purpose cleaner and Lime away cleaner.
At time of visit, PreK classroom and school-age classroom were combined.
In the twos room- Paper towel was on the cubby shelf and not on a dispenser. Kitchen had paper towel rolls on counters without being on a dispenser.
At time of visit, infant classroom ratio had 7 infants and 1 caregiver present. Toddler classroom ratio had 10 toddlers with 1 caregiver present.
Toddler restroom contained plunger, toilet brush and toilet training seat on the floor.
At time of visit, 7 month old infant sleeping on his stomach on the floor.
