A provider that cares for not more than sixteen (16) children at a facility where the provider operates a child care program shall maintain a ratio of children to caregivers in the same proportions as specified in the child to staff ratio requirements.
In both men and women’s bathroom, have toiles that are not working. In the men’s restroom, light not work.

A childcare provider shall have a written policy prohibiting: -use of tobacco, unintended use of toxic substances, use (homes) of alcohol; use or possession (centers & ministries) of alcohol; and use or possession of illegal substances in the facility where child care is operated when childcare is being provided.

Folding chairs stacked in hallway between infant room and toddler/ two room, not safe. Base trim peeling and section of base trim missing in hallway between infant room and toddler/two’s room. Toddler/Two’s: Bookshelf is wobbly and not safe. Gym: extension cord on the ground on the left-hand side by restrooms. Hallway leading to the outside play are is currently used as storage and is not safe.

A provider shall provide for a safe environment by ensuring that no conditions exist in or on the grounds of the facility where a provider operates a child care program that would endanger the health, safety, or welfare of the children, including ensuring that the following items are placed in areas that are inaccessible to children in the providers care: Poisons, chemicals, and cleaning materials
The provider shall make available daily activities appropriate to the age, developmental needs, interests, and number of children in the care of the provider; Both active and quiet play. The provider may include the use of safe, age-appropriate toys, games, and equipment for indoor and outdoor play.
