Ratio was not maintained. During naptime in Rm 166 Kindergarten specialist observed one staff with seventeen children with youngest age being confirmed to be four-years-old. Ratio was observed during nap time and confirmed that there was not 50% of the children sleeping. 1:17 The Infant 2 classroom Rm 160 did not have the proper number of qualified caregivers directly caring for infants. They had one qualified caregiver with eight infants with reported ages all under twelve months of age and the required ratio for that group is two. 1:8
At time when incident occurred staff and director self reported that staff was not providing appropriate handwashing hygine while working within the toddler classroom. Staff was reported to reach in snack bag bare hands to get a cracker and coughing and sneezing without washing hands.

At the time of inspection, in room 163 there were foam steps on the diaper changing table.
Hazardous items and items labeled "Keep out of Reach of Children" were found accessible to children in the following areas: Rm 170 had a basket of diaper creams and a basket of sunscreens on a low shelf in the bathroom. Rm 166 had liquid white out and handsanitizer in a low drawer, a basket of sunscreen in a low drawer in the bathroom and a teachers purse on the floor of an unlocked closet. Rm 156 had a basket of sunscreens and a basket of diaper creams in the unlocked cabinet under the changing table. Rm 141 had sunscreen in a low unlocked cabinet, and a teachers purse on the counter. Rm 147 had teachers purses in a low unlocked cabinet, liquid white out in a low drawer, and Aquaphor lotion in a low drawer in the bathroom.
Hot water at all hand washing sinks was over 120 degrees F. Hot water is to be between 100 and 120 degrees F to avoid scalding.
In room 137 there was a toy baby bed with dolls, a toy shelf and an art easel in the bathroom. In room 170 there were toy cars in the bathroom. In room 163 there was a broom and dust pan in the bathroom.
