Water marks on the ceiling tiles in the classroom in classrooms Room 5, Room 1, Room 10, and hallway missing ceiling tile, and has water marks. Room 9 ( infants) Ceiling tile falling down. Water fountain in hallway pulling away from the wall and is loose.

In room 4: Behind the toilet, there is a hole in the wall next along the pipe causing the dry wall to be exposed. In room 3 Sink cabinet has water damage along the side of cabinet, and wood is exposed, and is not sanitizable. Paint is peeling behind the toilet causing the drywall to be exposed, and is not sanitizable. Ceiling tile in the bathroom has a large water mark.

Infant 2 classroom has extension cord with outlet adaptor attached hanging from the television mounted up on wall. Bottle warmer is plugged into this extension outlet adaptor hanging within reach and accessible to children.
