Provides adequate supervision at all times Reviewed with the provider that she needed to be in sight and sound of the children at all times.<br/> The worker observed a child in the kitchen watching another household member cooking pancakes next to the stove. When the worker alerted the provider of the safety issue, the provider than removed the child from the kitchen and secured the kitchen gate, blocking access to the kitchen.<br/> The worker noticed that a house pet, a dog was standing over the patio gate next to a child. The worker had the provider place the dog in a gated kennel away from the child care away, until parent's/guardians have given consent and are notified of the provider's pet. The worker went over and reviewed safety and health issues with provider.<br/>
Protective measures under outdoor play equipment The worker observed the provider's dog climbing over the patio gate onto the child care area were a child stood. The worker had the provider place the dog in a gated kennel.<br/> The area were the dog is housed is within the outdoor play area.<br/> The worker advised the provider to have this outdoor area be off limits until she notifies parents of the pet and obtains authorization for children to socialize with pet.<br/>
Fire warning device system in working condition At the time of the visit the worker observed that the smoke alarm was removed from the wall.<br/> The worker discussed the safety issue the provider and she installed the fire alarm back on the wall.<br/> The fire alarm was in working condition.<br/>

One caregiver with current certificates during operating hours The worker reminded the provider that her First aid/CPR certificate expired on January 21, 2024.<br/> The provider stated that she understood, and will be getting re-certified as soon as possible.<br/>
Provider and family members have physical exam The worker gave hard copies of the DHS 984- 2 household members to complete and submit back to me.<br/> All other household members in compliance.<br/>
Environment free of hazards The house pet, a dog was seen climbing over the patio gate to play with the a child in care. The worker had the provider place the dog in a gated kennel away from the child care away, until parent's/guardians have given consent and are notified of the provider's pet. The worker reviewed safety issues with provider.<br/>
Provider has control of rodents and insects The worker observed that there were flies in the kitchen and spoke to the provider about the need for sanitary conditions in food preparation.<br/> The provider did acknowledge that she would replace the patio screen door that was torn.<br/>
Written practices and policies reviewed with staff, volunteers, and/or caregivers upon hire, start of care, or start of residence and on an annual basis The provider's spouse and another household member are in need of safe sleep training with PATCH.<br/>

Remove soft bedding, bumper pads, and other objects from the crib or playpen The worker observed 2 playpens and within the playpens where; toys, a blanket, and a roll of aluminum foil.<br/> The worker reminded the provider that she needed to remove the items and that at no time should there be anything in the playpen except for a fitted sheet.<br/>
