One caregiver with current certificates during operating hours According to the DHS 957 Staff Sequence form, not all hours of child care are covered by at least (1) staff member having department-approved certification.<br/> <br/> <b>Note</b>: The Worker received the XXXXX letter which lists staff members who have completed the American Heart Association Pediatric First Aid, CPR, and AED course. The department cannot accept a letter in place of a certification.<br/> <br/> As such, please obtain and provide the Worker XXXXX American Heart Association Pediatric First Aid, CPR, and AED certification to meet this requirement.<br/>

Ages of children accepted §17-896-6.1-11 Statement of operation policies (a) A before and after school child care facility shall have written operation policies, which shall be available to the department, caregiver staff, and parents or guardians of the children for whom care is, or may be available.<br/> <br/> <b>In the Parent & Student Handbook, please include the ages of children allowed as specified on the license for the Before & After School Program.</b><br/> <br/> <b>The license issued to the program is for children over 4 years 8 months to under 12 years of age. If this is not the age that you are serving in this program, please inform the Worker.</b><br/> <br/> Please provide the amended handbook to the Worker for verification that this requirement is being met.<br/>
Provisions/needs of individual children In the Parent & Student Handbook, please include a policy which addresses <b>provisions which may be made for the needs of individual children.</b><br/> <br/> If you need clarification, please contact Worker.<br/> <br/> Please provide the amended handbook to the Worker for verification.<br/>

Admission or non-admission of sick and children with special needs Although child care programs operated by religious organizations may be exempt from certain provisions of the Americans with Disabilities Act (ADA), <b>any program that accepts federally funded child care subsidies or Preschool Open Doors (POD) funds is required to comply with ADA requirements as a condition of receiving those funds.</b><br/> <br/> <b>Therefore, there are two options that you may elect to do in order to meet the licensing rules:</b><br/> <br/> <br/> 1) If you continue to accept children that qualify for subsidies, you shall amend the Student and Parent Handbook to include a policy on admission of children with special needs by including the following statement:<br/> <br/> "It is this center's policy to not discriminate against persons with disabilities on the basis of disability, and to provide children and parents with disabilities an equal opportunity to participate in the center's programs and services, in compliance with state law and the federal Americans with Disabilities Act."<br/> <br/> OR<br/> <br/> 2) You may remain exempt from the ADA but you must include the following statement in the Student and Parent Handbook:<br/> <br/> "This program does not accept federally funded child care subsidy."<br/> <br/> <b>Note: If the ADA statement continues to be excluded, we will refer this matter to our program office for review, and your program may be deemed ineligible to receive child care subsidy and Preschool Open Doors (POD) funds until compliance is achieved.</b><br/> <br/> <br/> For either option, please provide the Worker the amended Student and Parent Handbook for verification.<br/>

Staff training, experience, and personal qualifications The following staff have deficient health and safety training:<br/> <br/> XXXXX - Has 14 out of 16 hours of on-going training in 5 topic areas. Training period expired over year ago on 2/07/2025. Please have XXXXX complete the remaining hours of training immediately and send the certificates to People Attentive to Children (PATCH) for processing.<br/> <br/> <b>IMPORTANT: in addition to the remaining 2 hours needed to complete the 16 hours for the period of 02/08/2024 - 02/07/2025, XXXXX must submit 16 additional hours in training certificates to PATCH to become current in health and safety training for the period of 02/08/2025 - 02/07/2026.</b><br/> <br/> XXXXX - Has 11.5 out of 16 hours of on-going training in 5 topic areas. Training period expired 8/15/2025. Please have XXXXX complete the remaining hours of training and send the certificates to PATCH for processing.<br/> <br/> <b>Reminder: XXXXX must 4.5 hours needed to complete the full 16 hours of training for XXXXX 08/16/2024 - 08/15/2025 period. Additionally, XXXXX must submit 16 hours of training certificates for the 08/16/2025 - 08/15/2026 period before the end of this period in order to meet staff training requirements in August.</b><br/> <br/> <br/> <b>This requirement will be met once PATCH has verified all certificates and the hours have been updated in our system.</b><br/> <br/> As a reminder, please submit all 16 hours of each employee's on-going training to PATCH each well before each individual's expiration date to avoid interruption in requirement. Additionally, any new staff member, or returning staff member with a 180 consecutive day break in service, must complete the initial health and safety assessment within 45-days of hire.<br/> <br/> <b>Health and Safety training is a mandatory requirement for all staff who provide care and supervision to children. Employees who do not complete the required training will be deemed unqualified and may not be counted toward staff-child ratios. If the exclusion of unqualified employees results in insufficient staffing to operate the program, the program's license may be affected.</b><br/> <br/> We appreciate your prompt attention to this matter.<br/>
Other policies §17-896.1-11 Statement of operation policies (c)(18) states that the facility shall have written operation policies, which shall be available to the department, caregiver staff, and parents/guardians. Those policies shall include sanitation practices.<br/> <br/> To meet compliance, <b>please amend the Parent & Student Handbook to include the facility's sanitation practices.</b><br/> <br/> Additionally, §17-896.1-11 Statement of operation policies (c)(20) states that written policies shall cover grievances, which shall include the department's information for parents to file a complaint.<br/> <br/> The Grievance Policy for Parents is provided on page 30 of the Parent & Student Handbook. <b>Please amend the policy to include the department's information following the school's internal procedure. The department's information should be listed as such: Child Care Licensing Unit 1, phone number (808) 587-5266.</b><br/> <br/> Please provide the amended handbook to the Worker for review.<br/>
