Building codes We received the Inspector's Report from Honolulu City and County Department of Planning and Permitting (DPP) on May 15, 2026.<br/> <br/> The report states that this site does not meet code requirements due to the following cited violations were cited:<br/> <br/> VI-0081077 Storage Shed<br/> VI-0081133 Covered Patio<br/> VI-0081163 Multiple Exit Doors<br/> VI-0081226 Not Following Existing Use Permit<br/> <br/> Once these violations have been corrected, and DPP has re-inspected, approved, and sent the Department of Human Services (DHS) an Inspector's Report that indicates your site meets Code requirements, this deficiency will be resolved.<br/>

Safe sleep certifications 17-895.1-96(b) Program requirements.<br/> <br/> Safe sleep certifications have expired for the following staff members: XXXXX. Please have the above-mentioned staff take safe sleep training immediately and submit certifications to PATCH.<br/> <br/> <b>*As a reminder, safe sleep certifications shall not be expired as it may affect the health and safety of infants in your care. Safe sleep trainings shall be completed and submitted to PATCH by April 30, 2025.</b>*<br/>
Log of emergency preparedness drills Please provide your current school year log of emergency preparedness drills.<br/>
Emergency exits Provide us with photographic evidence (not close-ups but wide-angle photographs so we are able to confirm the location) of all emergency exits in the facility, so we may ensure it is unobstructed.<br/>
Fire extinguisher Please provide photographic evidence (not close-ups but wide-angle photographs so we are able to confirm the location) of an unexpired ABC multi-purpose type fire extinguisher for every room where infants and toddlers are cared for.<br/>
Daily nutritional needs Provide photographic evidence (not close-up but wide angle so we may determine where it is posted) of a list of food items that are provided by your facility and posted in a prominent place.<br/>
Physical arrangements for sick infants and toddlers Provide photographic evidence (wide angle photographs not close-ups to confirm the location of the items) of the physical arrangements for infants and toddlers, who become ill after arrival at the facility.<br/>
Operating accessible telephone Please provide written attestation that your site has an operating telephone (including telephone number) in case of emergency, that is accessible to all staff.<br/>
Emergency telephone access Provide us with a plan for accessing a telephone when child care is provided somewhere other than the usual location where care is regularly provided.<br/>
Written disaster plan Include the following in your disaster plan and provide us with updated plan:<br/> <br/> 1. Providing for the needs of children with disabilities and children with special needs during an emergency; and<br/> <br/> 2. Providing for the needs of children with chronic medical conditions during an emergency.<br/>
All caregivers meet qualifications Per HAR 17-895.1, all staff must have 16 hours of on-going Health & Safety trainings in at least two different topic areas. Training has expired for three employees XXXXX, XXXXX, and XXXXX.<br/> <br/> Training certificates shall be provided to PATCH, who shall update this training information for each caregiver.<br/> Please maintain this for all staff going forward.<br/>
