In violation of Hawaii Revised Statues §350-1.1 Reports (a)(5) by not reporting the alleged abuse by the former staff member. 350-1.1 (a)(5) Reports. Notwithstanding any other state law concerning confidentiality to the contrary, the following persons who, in their professional or official capacity, have reason to believe that child abuse or neglect has occurred or that there exists a substantial risk that child abuse or neglect may occur in the reasonably foreseeable future, shall immediately report the matter orally to the department or to the police department:<br/> Individual providers of child care, or employees or officers of any licensed or registered child care facility, foster home, or similar institution."<br/> <br/> Please adhere to the following corrective actions:<br/> <br/> 1. The regional director and the entire St. Timothy's Academy Preschool Program staff, volunteers, and substitutes, shall complete Child Welfare Services (CWS) Mandated Reporter Training (please email Sharon Adric, MSW at [email protected] to schedule in-service trainings and PATCH training - Child Abuse and Neglect.<br/> 2. Provide certificates of completion of CWS Mandated Reporter Training, and PATCH coursework no later December 31, 2024.<br/> 3. Director XXXXX and Regional Director XXXXX shall review annually the Child Welfare Services Mandated Reporter Guidelines.<br/> 4. Director shall provide the department with written attestation that she will review the Child Welfare Services Mandated Reporter Guidelines (enclosed), Kamaaina Kids Appropriate Touch Policies and Bathroom Guidelines at least annually with all staff, including substitutes.<br/> 5. Regular license will be replaced with a provisional license for a 6-month period effective June 28, 2024.<br/>
